If your child needs prescription medications at school, a medication authorization from will need to be signed by a parent/guardian and completed by your pediatrician.
Over the counter medications may be self-carried by your child in their backpack in middle school as long as a parent/guardian signs the self-carry form and returns it to the nurse. Just a reminder that the clinic does not stock medications.
If your child has a medical condition, the proper form must be signed by a parent/guardian and completed by the child’s doctor.
School Clinic
If your child calls or texts you from their phone/watch and states they are not feeling well please request they go and see the nurse. From the clinic we can discuss next steps on your child’s return to class or pick up from the clinic.
