Enrollments, re-enrollments and change of address requests must be completed on the online form and include the required documentation.
Health Documents required:
The health documents required for New Student Enrollment are:
- Birth Certificate
- Physical within the past year
- Immunization records – FL 680 FORM Any student who is new to the Florida school system must have up-to-date immunizations recorded on this form. The form may be obtained from the St. Johns County Health Department at 904-825-5055 or from a private physician. Note: for 7th and 8th grade students, TDaP vaccine must be on the form
The health documents required for Re-Enrollment are:
- 680 Immunization form with TDaP vaccine if students are entering 7th and 8th grade
Proof of Residency Requirements:
St. Johns County School District requires detailed proof of residency provided by a parent/guardian. All documents must be current, valid, and include the residential address used for enrollment. You MUST be a St. Johns County resident to enroll your child. Residency requirements can be found at http://www.stjohns.k12.fl.us/student/residency/information/.
Guardianship:
A guardian other than parents must provide the court documentation or have an updated birth certificate showing guardianship to enroll a student. We cannot accept Power of Attorney as guardianship.
